SAN JOSE, Calif. - A claims administrator and health insurer on Jan. 11 sued multiple ambulatory surgical centers (ASCs) in California federal court, alleging that the defendants fraudulently billed the plaintiffs for out-of-network services provided to plan members (Connecticut General Life Insurance Co., et al. v.
