Hello,

This is an important reminder regarding the loading process for new cases within the In re: Fosamax Litigation in the NJ Superior Court Atlantic County Civil Division E-Service on File & Serve.

Recently, we have had a number of firms within the Fosamax litigation searching for cases that are not available online in the Fosamax litigation because the cases have not been loaded onto File & Serve. If you have cases to load into the Fosamax litigation, please follow the guidelines below in order to have your cases loaded as quickly and efficiently as possible.

To have a case loaded onto File & Serve:

Download the In re Fosamax Case Load Template found HERE.
Fill in the requested Case and Party information.
Please enter the full name of each party on a separate line (including plaintiffs)
For Plaintiffs, First and Last names are required.
Attorney information for all Plaintiffs is required.
Attorney information for defendants is required, unless the party is unrepresented or counsel is unknown.
If the Party is unrepresented or counsel is unknown, please enter Firm TBD into the column for firm name.
Once complete, please forward the spreadsheet to Colleen O'Shea at Fox Rothschild (coshea@foxrothschild.com). She will review the data and work with File & Serve to have the case loaded onto File & Serve.

PLEASE NOTE: Any spreadsheet not filled out properly will be returned to the plaintiff firm for review and could delay the loading of the case into the litigaton.

This process is effective immediately.

All New Cases in the In Re: Pelvic Mesh/Gynecare Litigation should be submitted using the Pelvic Mesh / Gynecare New Case Template to our Data Integrity team using the following email address: lngcl-efile-di@lexisnexis.com.

Please CLICK HERE for a copy of the Pelvic Mesh / Gynecare New Case Template. This template is also located on the File & Serve Resource Center on the Rules and Procedures pages for New Jersey.

When filling out the Pelvic Mesh / Gynecare New Case Template, please remember that, in general, only the plaintiff information is necessary. This litigation uses a standard defense list so, unless you have additional defendants (other than Johnson & Johnson or Ethicon Inc.), you do not need to provide defendant information.

However, if you do have defendants other than those on the standard defense list, you must supply those defendants and their counsel information on the New Case Template. Also, please keep in mind that if the template contains incomplete plaintiff data, the case loading process may be delayed in order to obtain necessary information.

NOTE:

This process is only for cases in the Superior Court of Atlantic County, New Jersey In Re: Pelvic Mesh/Gynecare Litigation. At this time, it does not apply to any other Pelvic Mesh litigation.

Important changes to the Case Load Process for the File & Serve In Re: Reglan litigation in the Superior Court of Atlantic County, NJ.

This process is effective immediately.

When your firm has a new case to enter into the In Re: Reglan litigation on File & Serve, please take the following steps:

Using the new case load template, please enter all case data into the spreadsheet and save a copy for your records. Please Note that you can use the same spreadsheet to add multiple cases onto File & Serve.
- Case Data MUST INCLUDE the following criteria in order for the case to be loaded into File & Serve.
  - Case Number
  - Case Name
  - Party Information (Company Name or Party First and Last Name)
  - Party Type (Plaintiff or Defendant) **NOTE: Both Plaintiffs and Defendants must be listed in order to load cases onto File & Serve
  - Attorney Information (First and Last name) - If you do not know defense counsel information, enter TBD in the Last Name column.
  - Attorney Firm Information (Firm name, address, phone, email). **NOTE: Plaintiff attorney/firm information is necessary to upload cases, but for Defendants, you can leave these fields blank if you do not know what firm or attorney represents the party.
Email the completed spreadsheet to nj.reglan@lexisnexis.com, requesting that the case be loaded into File & Serve. If you have questions or comments, please feel free to enter those into your email, as well.
Once File & Serve has received your request for a new case, you will receive a confirmation email stating that File & Serve is reviewing the spreadsheet.
- If there are errors in the spreadsheet and we need to send it back to you or contact you with questions - this will delay the loading of the case into File & Serve.
Once the spreadsheet has been reviewed, the new case will be loaded onto File & Serve and you will be notified that it is available for use.
Case Load Templates that are properly filled out and submitted to nj.reglan@lexisnexis.com, should be available online within 48 hours.
