When filling out the In Re: Specially Assigned Mesh Implant Cases Template, please keep the following points in mind:

When entering plaintiff information, you need only list the word "Plaintiff" not the plaintiff's whole name. Generic Plaintiff designations are used for this litigation so party names are not necessary.
You must include Plaintiff's attorney and firm.
This litigation uses a standard defense list so, unless you have additional defendants (other than Boston Scientific Corp or the Massachusetts Superior Court of Middlesex County), you do not need to provide defendant information.
If you have additional defendants (other than those listed on the standard defense list), you must include them on the template in order to have them listed on the case. Please include attorney in firm information, when available.

Please keep in mind that if the template contains incomplete data, the case loading process may be delayed in order to obtain necessary information.

NOTE:

This process is only for cases in the MA Superior Court of Middlesex County In Re: Specially Assigned Mesh Implant Cases litigation. At this time, it does not apply to any other Pelvic Mesh litigation.
