    A new Web-based client interview form can save you and your staff hours of work in preparing bankruptcy cases. Direct clients to this new site to enter their contact, real property, personal property, debt, income, expense, dependent, occupational, and other important case information. Once a client submits the form, you will receive an e-mail notification and can then, for $6, download and import the answers to the Collier TopForm™ program.

    Collier TopForm™ Web-Based Questionnaire: How It Works

  • You ask your client to visit http://bankruptcy.lawyers.com/ to complete a Web-based bankruptcy questionnaire, where the client will be directed to enter the applicable case information.
  • Once your client has completed the Web-based bankruptcy questionnaire, you will receive an e-mail notification advising you to download the completed questionnaire for $6 (plus tax, if applicable).
  • Upon downloading the completed questionnaire then importing it into the Collier TopForm Program, the answers will flow directly into the applicable schedules and forms. Note: This product is for "Collier TopForm Web-Based Questionnaire" registrants only.
