Caradigm and MEDai Align to Help Healthcare Organizations Manage Risk, Optimize Care and Improve Population Health

October 16, 2013 — Caradigm to integrate MEDai’s industry-leading predictive clinical analytic solutions with data aggregation and care management solutions

Caradigm announced today it has signed an agreement with MEDai, a leader in predictive clinical analytic solutions, designed to give healthcare organizations the comprehensive set of tools they need to effectively manage risk, optimize care management, and improve the health outcomes of populations. MEDai is a LexisNexis company.

Under the agreement, Caradigm will enrich its population health solutions with the predictive clinical analytics capabilities of MEDai’s solution, enabling customers to stratify patient populations according to severity, disease state, cost or risk profile. This will allow risk-bearing organizations to use their network of resources to focus care on those who will benefit most, helping create a positive impact on overall health outcomes and total cost of care. When coupled with Caradigm® Care Management software, providers can develop personalized care plans that specify problems, goals and interventions, and manage patients and conditions through timely interventions enabled by near real-time, event-based alerts.

MEDai’s solution uses statistical algorithms to identify and risk-stratify patients based on a wide range of factors, including: severity of illness, patient motivation and compliance with evidence-based care guidelines, risk of inpatient admission or emergency department visit, and the total cost of care based on the number of conditions, prescriptions and type of care required.

Customers gain the following insight about populations and individual patients:

  • A total risk score, with the ability to change the relative weighting of the scores that make up the aggregate score
  • Patient stratification based on risk drivers, i.e., the specific elements causing a patient or cohort to be ranked at a certain risk level
  • The directional trend of the risk drivers for that patient or cohort (increasing or decreasing)
  • Trends, e.g., medication adherence, that can be addressed across multiple patients
  • Costs associated with risk factors and no intervention
  • Quantification of possible savings associated with each intervention

This data is presented within Caradigm Care Management, helping care managers take the appropriate actions to put patients on track to improving their health and reducing the cost of care.
