Medical Plan
We provide our employees with medical benefits for themselves and their eligible dependents. Employees and the Company share in the cost of medical coverage. Costs vary depending on the plan chosen, whom you choose to cover and your benefit earnings. When you or a covered family member needs medical care, your medical benefits can provide valuable financial protection. There is no pre-existing condition clause or limitation in any of the medical plans we offer.

We provide our employees with dental benefits for themselves their eligible dependents. Employees and the Company share in the cost of dental coverage.
Our dental benefits are provided through Aetna Dental. With our dental plan, you can go to any dentist you wish, however, if you choose a dentist in the Aetna network, your dental benefits will go further.
Our dental plan covers the following dental services:
Preferred Provider Organization (PPO):

Vision Plan
We offer our employees vision benefits through our agreement with VSP. Employees can choose from two options:
- Vision Service Core Plan and Buy-Up Option:
- Vision exam every calendar year
- Glasses: New lenses every calendar year; allowance for one pair of frames every other calendar year

- Contact lenses: allowance that is applied toward both your contact lens exam AND your contact lenses every calendar year
- The buy-up doubles your benefit – all the features of the core plan plus the option of additional frames every other calendar year, or glasses lenses or contact lenses every calendar year

Flexible Spending Accounts
We offer employees the opportunity to contribute pre-tax dollars to one of two Flexible Spending Accounts (FSAs) – Health Care and Dependent Care. FSAs offer a convenient, tax-effective way to pay for eligible health care and dependent care expenses that are not covered or reimbursed under any other plan. Employees allocate a portion of their salary to one or both accounts before federal income and Social Security taxes – and in most cases, even state taxes – are taken out.
By participating in a FSA and deducting your contributions before taxes are taken out, your taxable income is reduced. These before-tax contributions do not affect other pay-related benefits. Future Social Security benefits may be based on a slightly lower wages as a result.
- A convenient, tax-effective way to pay for health care or dependent care expenses that are not covered or reimbursed under any other plan.

The Salary Investment Plan (SIP) has been established as an easy way for employees to take charge of their future retirement savings. Employees can choose to save money on a before-tax and/or an after-tax basis.
- Option of before-tax, after-tax or Roth after-tax contributions, up to 75% of pay, via payroll deduction; subject to IRS limits

(PTO)
The PTO policy includes annual vacation, sick and personal days in one bank from which you may draw for any
reason, as long as you have manager approval.
The PTO schedule is as follows:
0 < 2 years of service = 18 days per year
2 - 6 years of service = 23 days per year
7 - 19 years of service = 28 days per year
20+ years of service = 33 days per year
Buy-up option for 5 more days through paycheck reduction

Volunteer Days
- 2 days per calendar year for volunteer work at a nonprofit organization of your choice
